Miller Vein Friends & Family Program – Terms & Conditions 

Miller Vein patients (“Patient”) may refer friends or family members (“Friend”) for VeinErase® evaluations and treatment. Each Patient’s record includes a unique identification code that will be referenced by the Friend when booking a VeinErase consultation. 

  1. An evaluation may be required before VeinErase® treatment. Miller Vein will assess treatment needs and determine a treatment plan accordingly. Miller Vein makes no guarantee that treatment will be necessary or will be provided.  
  1. Due to legal constraints, the free evaluation offer cannot be extended to Medicare or Medicaid beneficiaries or other recipients of federal or state health care benefit programs. 
  1. Offer can only be applied to cash-based patients. No insurance plans will be accepted for treatment under this promotion including MESSA, Medicare, and Medicaid. 
  1. Discounts apply for new appointments only. Discounts cannot be combined with any other rewards codes, discounts, offers, or promotions.  
  1. Discount can be used for new VeinErase® procedures only. It cannot be applied to previous treatments or purchases. It cannot be used toward a medical procedure, compression stockings, or any other purchase.  
  1. The discount is non-transferrable, has no cash value and cannot be redeemed for cash. 
  1. Discount will be awarded when a referred Patient arrives for their first screening appointment.   
  1. Miller Vein will contact the Patient that referred the Friend to advise they have received their promotional discount. 
  1. Miller Vein reserves the right to limit or cancel any discount due to ineligible use or abuse of the promotional terms.
